Yoğun bakım Ünitelerinde 24 saat nöbet şeklindeki çalışma modeli süregidiyor! Bazı hastanelerde, 16 saatlik çalışmanın da çok yıpratıcı olduğu, sağlık çalışanlarından yöneticilere yönelen itirazlar bildiriliyor. Bir taraftan yeterli korunma malzemesinin ve tanı malzemesinin yetmemesi, bir taraftan kendileri ve bakım verdikleri hastalar için aşırı kaygılı ruh hali, hemşire ve bakıma yardımcı personelin artan hasta sayısına yetmemesi, hekim grubunun, tedavi planını çalışma odalarından reçete etmesi, hasta yanına gelip girişimlere katılmaması, hasılı sağlık ekibi diyeceğimiz bir ortaklık olmamasının yarattığı gerilim…. Sonunda covid 19 bulaştı mı merakı içinde test olmayı bekleyen, hatta pozitif olsa da izolasyona alınsam belki dinlenirim diyen genç ve orta yaşlı hemşireler!
- Ben yetkili olsaydım: Çalışma süresini, “yoğun bakım ünitelerinde” 6 saat, kontrol altındaki hastaların yattığı servislerde 8 saatle sınırlandırırdım ; hatta hemşirelik işlerine tahsis edilmiş görevliler 24 saat ve uzun saatler çalışmayı isteseler bile izin verdirmezdim. Uluslararası önerilere uyardım. Büyük şehirlerde özellikle kadın görevlilerin evlerine güvenli ulaşım derdi olduğundan, sağlık kurumunca ya da belediyelerce, her sağlık görevlisine, işden- eve ulaşım desteği sağlanması zorunludur.
- Gönüllü çalışacakları devreye sokardım, çünkü getir götür, temizlik işlerini yapacak eleman bile bulunmuyor, yetmiyor! Kimler olabilir? Üniversitelerin Hemşirelik Bölümü son sınıf ve tıp fakültesi son sınıf öğrencilerinden (özellikle 2019-20 yılının 1. döneminde YBÜ-Acil- İnfeksiyon Servislerinde staj yapmış olanlar) istekliler hemen davet edilebilir (nitekim ATT’den, anestezi teknikerlerinden test materyalinin alınması için görevlendirilmeler yapılıyor). Burada dikkat edilecek husus gönüllünün yerliliğine uygun iş tanımının/ sınırlarının belirlenmesi ve buna sıkı sıkıya uyulmasıdır.
- Hastaların tanı- tedavi-bakımı için çalışanların ortak bakım planları hazırlamalarını ister, kurumda bunu izleyecek vaka yöneticileri (uzman hemşireler) görevlendirirdim. Bunu yapmış sağlık kurumlarından danışmanlık, bilgi alışverişi isterdim.
- Hemşirelerin ve hekimlerin çalışma ortamının konforunu artırırdım; bunda gönüllüleri seferber ederdim. (YBU' nin yakınında dinlenme, mola verme alanı, terapötik bir ortam, masaj makineleri, sıcak içecek malzemesi, cay- kahve makinesi ve atıştırmalıklar, taze meyve suyu, , hafif müzik, aileleriyle görüntülü görüşme imkanı, hatta lüzum halinde,iş çıkışı kalacağı konteyner vb yerler, banyo yapma imkanı, dertleşecek klinik psikologlar, hoşlanacakları şekilde(halkla ilişkiler) karşılama ve uğurlama, küçük armağanlar, teşekkür belgeleri, isteklerini okuyup yanıtlarken nazik, umut verici bir üslup, sosyal ihtiyaçlarına destek vermeye hazır sosyal hizmet görevlileri.....sekreterlik, rehberlik yapacak gönüllüler… (siz çoğaltın?)
- Demek ki inovatif ve dost, sponsor zinciri uzun, kriz yönetimini bilen ya da hızla öğrenen yönetici personele ihtiyaç büyük; gerçek yöneticiler “hizmet alan, hizmet veren, kurum” beklentilerini, ihtiyaçlarını iyi dengeleyenlerdir; burada hizmet verenlerin anahtar olduğunu unutmadan.
- Yakın gelecekte, Haziran 2020’de Sağlık Kurumlarımızın yönetici kadrolarının değerlendirilmesinin yapılıp, “devam veya tamam” kararı verilirken kriz yönetiminde, insan kaynakları yönetiminde, personel geliştirme konusunda yetkinliğin aranması elzemdir.
- Sağlık Bakanımız, sıkıntı yaşayanlara, kendisine ulaşmak isteyenlere açık olduğunu bildirdiğine göre, tükenmeden ve isyan etmeden durumunuzu bildiriniz. Sağlık Bakanlığımız, Sağlık Kurumları Yönetimi Uzmanlarının rehberliğini temin edebilir, S.B.’nın sağlık kurumları denetçilerini seferber edebilir.
Mezunlarımızın bunu okuyacak zamanları yoktur; olsaydı, hastaları için uygun ve elzem olanın benzerini, günlük yaşam gereksinimlerinin karşılanmasını, en hayati "korunma malzemesi"nin teminini, test edilme fırsatı olmasını, hekimler ve destek olabilecek sağlık görevlilerinin de ünite içinde tanı- tedavi- bakım işlemlerine katılmalarını ve başarı varsa bunun onurunun paylaşılması... gibi olmazsa olmaz hususları dile getirirlerdi... Daha iyisi, cesaretle, onların konuşmasıdır.
