Miyop ve şaşılık çocuklarda "salgın" gibi arttı

İstanbul Üniversitesi-Cerrahpaşa (İÜC) Cerrahpaşa Tıp Fakültesi Göz Hastalıkları Ana Bilim Dalı Öğretim Üyesi Prof. Dr. Hüseyin Yetik, koronavirüs salgınıyla çocuklarda artan miyop ve şaşılığı "pandemi" olarak nitelendirdi.

Prof. Dr. Yetik, yaptığı açıklamada, son yıllarda çocuklarda miyop ve şaşılıkta yaşanan artışa dikkati çekti.

Miyobun halk arasında "uzağı görememe" diye bilindiğini aktaran Yetik, şaşılık ve göz kaymasının ise birbirlerinden farklı gibi algılandığını ancak aynı durum olduklarını ifade etti.

Tüm dünyada kliniklere müracaat eden çocukların şikayetlerinin ortak olduğunu ve bunun uluslararası toplantılarda paylaşıldığını dile getiren Yetik, " 'Dünyada koronavirüs pandemisiyle beraber bu hastalıkların da pandemisi oldu' şeklinde ifade ediyoruz; birincisi miyop, ikincisi de şaşılıklar, göz kaymaları. Nedeni yoğun şekilde yakına bakmak, pandemide eğitimlerin uzaktan ve bilgisayardan yapılmaya başlanması, çocukların daha uzun süreli tablet, telefon, bilgisayar, televizyon karşısında eğitim almak üzere zaman geçirmeleri. Bu hem miyobu tetikledi hem de göz kaymalarını." değerlendirmesini yaptı.

Hüseyin Yetik, telefon ve tablet gibi cihazlarda kullanıcı yaşının çok düştüğünü vurgulayarak, şöyle devam etti:

"Çocuklarda şaşılık yüzde 80-90 oranında ortoptik yaklaşımla yani gözlük takarak, kapama tedavileriyle ameliyata gitmeden çözülebilir. Bu dönemde müşahede ettiğimiz önemli şeylerden biri ortoptik yaklaşımla düzelttiğimiz, yolunda giden tabloların da artık ona yanıt veremez olmaya başladığı. Çünkü gözü kaydırıcı faktör olan aşırı yoğun bakma o kadar aşırılaşıyor ki artık ortoptik yaklaşımınızı da aşan bu süreçte çocukların gözlük uyumu bozuluyor, gözlüğün üstünden bakıyor. Bir taraftan çocukların, ailelerin sağlık kuruluşlarına müracaat sıklıkları azalıyor."

Göz kontrolleri yeni doğan döneminde başlamalı

Yetik, her yeni doğanın 1 ay içinde göz muayenesinin yapılması gerektiğini, doğumun ardından bir hafta içinde yeni doğan doktorunca bebeğe karanlık bir odada gözde doğumsal bir problem olup olmadığının ölçülmesi için kırmızı yansıma testi yapıldığını anlattı.

Doğum sonrası en erken 6 ay, en geç de 1 yaşında çocuğun muayene edilmesi gerektiğine işaret eden Yetik, ebeveynlerin, "Çocuğumun gözünde hiçbir sorun görmüyorum, yerdeki küçücük karıncayı görüyor, uçan kuşu gösteriyor." düşüncelerinin doğru olmayacağını, çünkü çocuğun bunu bozuk olmayan tek gözüyle de yapabileceğini ifade etti.

Prof. Dr. Yetik, 2,5-3 yaş sürecinin, bu yaştaki bir çocuğun kendini ifade edebilmesi sayesinde görme oranı hakkında net fikir edindikleri bir dönem olduğuna dikkati çekerek, "Çocukta o vakte kadar teşhisi atlanmış bir göz tembelliği varsa olabildiğince az sekelle düzeltilebilir olduğu dönemdir 2,5-3 yaş. Dolayısıyla 3 yaşında da görülmelidir." dedi.

Yetik, 40 yaşından itibaren de sağlıklı tüm bireylerin her yıl en az bir kez muayeneye gitmesinin önemli olduğunu vurguladı.

Fotoğraflarda gözdeki beyaz yansıma sağlık sorunu habercisi olabilir

Prof. Dr. Hüseyin Yetik, kendilerine başvuruların genellikle çocuktaki göz problemleri okul taramalarında fark edildikten ya da çocuğun, okulda tahtayı takip edemediğini anlaması üzerine yapıldığını aktardı.

Göz kaymalarının fotoğraflarda da farkedilebileceğini söyleyen Yetik, şöyle konuştu:

"Fotoğraflarla ilgili dikkate sunulacak bilgilendirici bir başka şey de göz bebeklerinin kırmızı ya da beyaz refle (yansıma) vermesi. Modern fotoğraf makineleri kırmızı göz görüntülerini siyaha dönüştürürler. Fotoğraf çekildiğinde göz bebeğinin ortasında kırmızı yansıma varsa burada sorun yok. Ama bir gözde kırmızı yansıma, öbüründe kırmızı değil beyaz yansıma varsa bu ciddi bir göz problemiyle ilgili olabilir. O yüzden, ailelerin basit bir fotoğrafta bunu fark etmeleri bile önemlidir. Beyaz refle sağlıksızlıktır, retina dekolmanı (ayrılma), gözde tümör, katarakt, gelişimsel göz anomalisi anlamına gelebilir."

Göz sağlığını korumak için öneriler

Prof. Dr. Yetik, göz sağlığını korumak için yapılması gerekenleri şöyle sıraladı:

"20-20-20 kuralı var. Bilgisayar karşısında otururken her 20 dakikada bir, 20 saniye süreyle 20 feet (6 metre) ve uzağına bakarak gözü dinlendirme. İyileştirici faktörlerden biri bu. Bir diğeri miyobun gerçek anlamda artışını durduran, uluslararası çalışmalarla kanıtlanmış hemen hemen tek modalite, bilgisayar karşısında kapalı ortamda, odada değil daha çok dışarıda, gün ışığında vakit geçiren çocukların miyop artışlarının daha azaldığı belgelenmiştir. O yüzden, çocukları artık kapalı ortamlarda, ev ortamlarında, bilgisayar karşısında değil daha çok dışarıda zaman geçirmeye sevk etmek gerekiyor."

Yetik, gençlerde de son dönemde kullanılan zayıflama ilaç ve çaylarının da gözde ciddi yan etkilerinin gözlemlendiği uyarısı yaptı.

Prof. Dr. Yetik, çok kuvvetli ve göze direkt vuran ışıklardan olabildiğince uzak durulması, okuma yaparken metnin aydınlatılması ve ortamın karanlık olması, göz ile ilgili şikayet olsun ya da olmasın rutin taramalar için göz hekimine müracaat edilmesi gerektiğini sözlerine ekledi.
