Aile hekimleri 8 Şubat’ta neden Ankara'da?

Son zamanlarda giderek artan sağlıkçılara yönelik şiddet vakaları ardından, AHEF 8 Şubat'ta Ankara'da büyük bir miting kararı almıştı. Medikritik ekibi olarak AHEF(Aile Hekimleri Dernekleri Federasyonu) basın yayın komisyonu üyesi, aile hekimi Dr. Cumhur Özdemir ile hem aile hekimlerinin sorunları üzerine hem de hazırlanılan miting hakkında bir röportaj gerçekleştirdik.

Sağlıkta şiddetin son dönemde bu denli yaygınlaşmasının kaynakları sizce nedir? 

- Sağlıkta şiddetin çok yaygın olmasının bizce başlıca nedeni caydırıcı cezaların olmamasıdır. Doktorlara veya diğer sağlık çalışanlarına karşı sözlü ya da fiziksel şiddet uygulayan insanların cezalandırılmadığını ya da şartlı salıverildiğini; soruşturma olmadığını ya da cezanın para cezasına çevrildiğini görüyoruz. Bu da şiddet uygulayanların cezalardan korkmamasına yol açıyor, şiddet tekrarlanabiliyor. Tabii ki tek sebep bu değil, insanların şiddete yönelimi var. İsteklerinin karşılanmadığını görünce birçok insan sözel ya da fiziksel şiddete başvuruyor, şiddetin çözüm getireceğini zannediyorlar. Çekinecekleri cezai yaptırımlar da olmayınca, maalesef bizler şiddete maruz kalabiliyoruz. Hukuki alt yapı sağlam hazırlandığı sürece, sağlık okur yazarlığı da arttığında sağlıkta şiddetin azalacağına inanıyoruz.

Aile hekimlerine usulsüz işlem yaptırmakta ısrar eden ve istediği yerine gelmeyince şiddete başvuran vatandaşlara tanık oluyoruz. Bu talepkârlığın nedenleri nedir? 

- İnsanlar aile hekimlerine çok kolay ulaşabiliyorlar. Bu kolaylıkla birlikte, her sorunun aile hekimliğinde çözülebileceğine dair bir algı var. Halbuki aile sağlığı merkezleri, birinci basamak sağlık hizmetleri dediğimiz özellikle koruyucu hekimlik, önleyici sağlık hizmetlerini yerine getirmek içindir. Bu hatalı anlayışla beraber hastaların aile hekimlerine yanlış yönlendirilmesi de söz konusu. Bir örnekle açıklayacak olursak; işe giriş raporları için tarafımıza yönlendirilen hastalar oluyor. Ancak bir insana işe giriş raporu verebilmek için, nasıl bir ortamda çalışacağını, ne iş yapacağını, o işin risklerini bilmek gerekli. Bu raporu insanların çalıştığı ortamını bilen ve gereken sağlık kontrollerini yapan iş yeri hekimlerinin vermesi gerekmektedir. Biz mevzuata uygun olan evlilik raporları gibi, askerliğe elverişlilik gibi raporları; gerekli sağlık taramalarını yapıp, gerekli gördüğümüzde de sevk ederek zaten çözüyoruz. Mevzuatta olmayan her türlü işlem usulsüz işlemdir. Talepler sadece raporlarla da sınırlı değil. Komşusundan bir ilacın iyi olduğunu duyup da yazdırmak isteyenler de oluyor, kimliksiz işlem yaptırmak isteyenler de... Hastaların bu taleplerini uygun görmeyip yerine getirmediğimizde de, işlerini zorlaştırmakla suçlanıp çeşitli hakaretlere, tehditlere, fiziksel saldırılara maruz kalabiliyoruz maalesef. 

GÜVENLİK ÖNLEMİ YOK

Peki ASM'lerdeki sağlık çalışanlarını şiddete karşı koruyan bir mekanizma var mı? 

- Hayır, hiçbir şekilde güvenlik önlemimiz yok. Hastalar ASM'lere istedikleri gibi girip çıkabiliyorlar. Buradaki tek koruyucu mekanizma kendimiziz. ASM'de bir tartışma, gürültü duyduğumuz zaman işi gücü bırakıp sesin olduğu yere koşarak müdahale etmeye çalışıyoruz. Herhangi bir şiddet olayında da beyaz kod protokolüne göre davranıyoruz. 

Aile hekimlerine yüklenen idari sorumluluklara örnekler verebiliriz misiniz? 

- Aile hekimliği sistemine geçildikten sonra, ASM'lerde aile hekimleri idareci olarak da çalışmaya başladı. Eskiden sağlık ocağı sistemindeyken binalar devlet binalarıydı ve doktorlar sadece görevlerini yapıyordu. Şimdiyse binaların kiralanması, demirbaş eşyaların temini, faturaların ve tıbbi giderlerin hesapları, ASM'de çalışan tıbbi sekreter, temizlik görevlisi, tarafımızca çalıştırılan hemşireler gibi personellerin maaşları ve sigorta primleri gibi giderlerin hepsini aile hekimleri yatırılan cari gider ücretleri içinden ödüyorlar. Yani aile hekimleri hem idareci hem muhasebeci hatta yeri geldiğinde tamirci bile diyebiliriz. Kamu hizmeti versek de özel bir işletme gibi çalıştırıyoruz ASM'leri.

SAĞLIKTA ŞİDDETE DUR DİYELİM

AHEF olarak 8 Şubat'ta Ankara'da büyük bir miting düzenleme kararı aldınız. Sonrasında pek çok sağlık kurumundan da destek ve katılım çağrısı geldi. Bu eylemin amacı ve talepleri nelerdir, bize kısaca bahseder misiniz?

- Son yıllarda sağlıkta şiddet olayları çok arttı ve sağlıkta terör boyutuna geldi. Biz de buna "dur" demek için sahaya inmek istedik. Meslektaşlarımıza kararımızı bildirdik ve çok ciddi destek gördük. Bu amaçla 8 Şubat'ta Ankara'da, bütün meslektaşlarımızla birlikte sağlıkta şiddete dur demek için meydanlarda olacağız.
